Self Pay Fee Schedule

*We are In Network with BCBS Commercial plans (all PPO plans, Blue Value POS plans and Blue Care HMO plans). We are not in network with BCBS EPO plans. Fees are subject to your individual plan: co-pays and co-insurance may apply.

• Initial Consultation: $250

• Follow-up: $125

• Brief Follow-up (medication/supplement management only): $75

• Extended Follow-up: $175

• Misc Provider Services (letter preparation, clinical phone calls/emails, etc): $25 per 15 minutes

• No Show Fee: Full cost of appointment type that was scheduled

• Late Cancellation Fee (<48 hours prior to appointment): $100

• Travel Fee (for home and office visits): based on mileage

• Labs/Clinical Testing: rates dependent on test and rendered by testing company

**All fees are due at the time of service and payable by credit card or debit card through our patient portal. We are currently out-of-network for most medical insurance companies. If you have out-of-network mental health benefits, we are happy to provide bills or records that are requested by your carrier for you to file for reimbursement. Please be aware some insurance carriers will not reimburse for virtual appointments, so you may need to verify with your insurance company. We are currently unable to provide services for patients who are enrolled in Medicare or Medicaid.
