Fee Schedule
Initial Consultation: $250
Follow-up: $125
Brief Follow-up (medication/supplement management only): $75
Extended Follow-up: $175
Misc Provider Services (letter preparation, clinical phone calls/emails, etc): $25 per 15 minutes
No Show Fee: Full cost of appointment type that was scheduled
Late Cancellation Fee (<48 hours prior to appointment): $100
Travel Fee (for home and office visits): based on mileage
Labs/Clinical Testing: rates dependent on test and rendered by testing company
*All fees are due at the time of service and payable by credit card or debit card through our patient portal. We are currently out-of-network for all medical insurance companies. If you have out-of-network mental health benefits, we are happy to provide bills or records that are requested by your carrier for you to file for reimbursement. Please be aware some insurance carriers will not reimburse for virtual appointments, so you may need to verify with your insurance company. Patients who are enrolled in Medicare or Medicaid are not eligible for reimbursement through these programs.
